Please send the following-

– Application card (if you have one) completed with owner & dog info.
– The Vet certificate showing current rabies info.
– Check for fee… PLEASE DO NOT ADD INTO TAX PAYMENT CHECK.
– Self-addressed stamped envelope for license to be mailed back to you.
